Issued by the NMC after you clear FMGE. This is the key document for every step that follows — registration, internship and PG.

Apply for Provisional Registration
Apply to ONE State Medical Council (you can be registered with only one at a time). Provisional registration is what lets you start your internship.

Get NOCs & internship allotment
Obtain NOCs (from the state Directorate of Medical Education and the hospital), then receive your internship allotment order to a recognised college/hospital.

Complete the 12-month CRMI internship
Do the Compulsory Rotating Medical Internship in an NMC-recognised Indian hospital. For students admitted after 18 Nov 2021, this 1-year Indian internship is mandatory even if you interned abroad.

Collect the Internship Completion Certificate
Issued on successful completion — required to apply for permanent registration.

Submit to your State Medical Council / NMC. Once granted, you are a registered medical practitioner in India.

With permanent registration you can practise legally in India and are eligible for NEET-PG (MD/MS/DNB).

You can register with only one State Medical Council at a time, and it directly affects your internship slot availability and waiting time. Some popular states have internship backlogs while others clear faster. Pick strategically — based on domicile, slot availability, and where you plan to practise.
Realistic timeline
Provisional registration is usually quick (a few weeks). The internship is a fixed 12 months. Permanent registration follows internship completion. Budget roughly 12–15 months from your FMGE pass to permanent registration — longer if your chosen state has an internship-slot waiting list.

The Compulsory Rotating Medical Internship is a full year of supervised, paid clinical training across core departments in an NMC-recognised Indian hospital — the same internship Indian MBBS graduates do.
Rotations
You rotate through Medicine, Surgery, Obstetrics & Gynaecology, Paediatrics, Community Medicine, Orthopaedics, Anaesthesia, Casualty and others — getting hands-on ward, OPD and emergency exposure.
Seat allotment
Internship seats for FMGs are allotted through your State Medical Council / state counselling, often using your FMGE result. Seats are finite, so popular states can have a waiting period.
Stipend
Interns receive a monthly stipend that varies widely by state and government vs private hospital — treat it as modest support, not income.
Recognised hospitals only
The internship must be at an NMC-recognised college/hospital for your completion certificate to count toward permanent registration. Don't accept an unrecognised placement.
Document legalisation: apostille & attestation
This is where many foreign graduates get stuck. Your foreign degree and transcripts must be officially legalised before Indian authorities accept them:
- Apostille (for countries in the Hague Apostille Convention) or embassy/MEA attestation (for others) of your MBBS/MD degree and transcripts.
- Certified translations if documents are not in English, notarised as accurate.
- Name consistency across passport, degree and NEET/FMGE records — even a small spelling mismatch can stall registration. Fix discrepancies early.
- Start the legalisation process before you leave the host country where possible — it's far harder to arrange from India.
