Russia hosts some of the oldest medical universities in the world. First Moscow State Medical University (Sechenov) founded in 1758 has been producing doctors for over 265 years, making it the oldest medical institution in Russia. Kazan State Medical University established in 1814 has 210+ years of legacy and predates many prestigious institutions globally. Saint Petersburg State Medical University (Pavlov First) founded in 1897 has 125+ years of excellence. These universities have survived political changes while maintaining academic excellence.

Older universities typically have larger teaching hospitals, more experienced faculty and established relationships with healthcare institutions. Sechenov achieves 28-32% FMGE rates, Kazan State reaches 40-41%, and Pavlov First maintains 25-30% success—significantly higher than the national average. Newer universities lack this historical data making their future performance uncertain.

However age alone does not guarantee quality. Some newer universities have invested heavily in modern facilities and innovative teaching methods. The best approach is considering establishment year alongside current FMGE results, infrastructure and student feedback.
